I will be travelling to Italy in 2018.  What is the proper procedure to obtain permission to operate?  I am confused regarding reciprocal agreements, designated countries, etc.  Thanks in advance.

Easy ... Italy is a CEPT country.
Reciprocal operating authority for U.S. Extra Class and Advance Class hams is automatic.
You just need to have a copy of your license (print out an "Official Copy" on line at the ULS), passport and a copy of the CEPT announcement document from the FCC.
