I think the idea of "saying" in your head in "dit-dah" form the letters and words that you see around you is an excellent one and it certainly helped me.  Another thing that has worked well for me especially moving from writing down the words to head copy, is using an MP3 player with random words and amateur expressions recorded from the excellent software "Just Learn Morse Code" or off air if I find a QSO at the right speed..  Record at the speed and separation between characters that you can head copy (at least to some extent) and it is surprising how quickly it gets easier.  I take mine to bed and have it going before I get to sleep.  Great way to bring on sleep too!
