Please excuse the bonehead question, but I have been out of it for a while.
I have moved, what form do I need to fill out so Fred Chuck & Charlie can make the address change on my license?
Where can I get said form?

Hi,

I know you asked for the form, did you know you can
do it from the fcc uls website ?

http://www.arrl.org/fcc/uls101.html

Some vec will accept and process the change for
a fee or you can do it online for free.

FCC Form 605.

If an ARRL member, you can go to an ARRL
VE session and they will do it free
on a NVEC 605. $12 otherwise.

If you do the address change through the FCC site, Low and behold, miracles of miracles, within a week or so (perhaps even less,) your change SHOULD also appear at QRZ, E-HAM, and all the other callsign servers.
