My rig has a "Tuner" button and a "Start" button. I'm using an FT-840. I'm guessing it would be the "Start" button?

No. TUNER activates the internal tuner and START will initiate the tuning process. Look for a button called MOX at top left extreme corner. That is the manual transmit keying switch (same as plugging in a CW key and closing the contacts to send). Put rig in CW mode and press MOX to transmit. Deselect MOX to switch back to receive.

When I tune using my FT-840, I put it in AM, reduce output power to a minimum, and press the "MOX" button to get a continuous carrier.

Try it with CW, MOX (TUNE) and reduced power. Less bandwidth and same effect.

In the absence of any modulation, there will be no bandwidth difference between AM and CW.  I guess I always put it in AM out of habit, though you are correct, CW would be the one to use.
