For some reason, I have lost my license. How do I go about replacing it?

Log in to the ULS. Over to the right on the page that lists your license is a link that says "Request Duplicate."

I've also seen a place to print it.  Don't remember where though, might be ARRL?

I should have included earlier the comment that the actual piece of paper from the FCC really shouldn't be that important. That's because the FCC defines your license as the entry in the ULS database, not the paper itself. But if you want/need an official duplicate (for example, as proof for an international reciprocal license), the ULS (and the process I listed earlier) is the only place to go.

If all you need is some other sort of printout, there are other options that may be more visually attractive (depending on your style). AE7Q will print a full-color replica of your FCC license. VanityHQ lets you create a certificate with your license info.
