Phone Patch Problems

I am new to this, but I thought I would post my problem.

I am trying to get an MJF 624D Phone Patch inline with an ICOM 706MKII so that I can offer two way comm out of Atlanta.

Since there is not a patch in and out connection on the radio, I have to wire it the alternate method as shown in the manual.

I tried to test it on the air today, but it didn't work well.    A fellow ham told me that I was not getting the same output level with the patch in as I was with the patch off.

When I finally got a level that was acceptable and not distorted, I tried to call someone and put her on the air with my friend via the patch.   She was not heard.  She could hear everything, but she could not be heard. (I was not using VOX mode...was manually pushing my mic button.)

Also, after about a minute, my phone hung up.  

Anyone have any advice on this patch?  Alternate solutions?
